This Aged Care facility is looking for a Registered Nurse with community home-based nursing experience to manage health care workers in both private homes and institutions.

Key Responsibilities:

• Selecting carers to join the Community Care at Home programme
• Conducting orientation, training and skills development workshops for community carers.
• Managing conduct and performance issues with carers as necessary
• Developing appropriate marketing strategies and partnerships for the Community Care at Home unit.
• Administrative processes including the issuing of panellist contracts, verification of timesheet and payments, (dealing with medical aids and medical facilities)
• Ensuring that quality assessment reviews from the client/patient are completed and that feedback is given in close liaison with Admin & Finance, prioritised and actioned as necessary
• Conducting detailed patient assessments either at the patient’s home or in a medical facility
• Conducting patient assessments prior to admittance to the Sub Acute facility (in conjunction with medical aid partners and programmes)
• Sending patient reports to medical aids
• Selecting carers for specific patients and ensuring that a full on-site orientation is conducted with the Carer at the commencement of the assignment
• Provision of clinical care to patients as required.
• Ability to do minor wound care.

Requirements:

• Must be a Registered Nurse with current SANC registration and membership.
• Must have 3-5 years of clinical practice relevant to community health and managed health care work experience a strong recommendation
• Valid driver’s licence
• Excellent communication skills, both written and verbal
• Ability to function effectively unsupervised and in high-pressure environments
• Ability to be on the road for 7 hours a day to visit patients and clients
• Ability to work closely with colleagues and stand in for admin if needed – To be a team player
• Ability to do call from home for a week and sometimes 2 weeks per month
• High levels of empathy
• Good staff and client management experience
• Working knowledge of disciplinary procedures, enquiries and grievances.
• Working knowledge of Oncology patients, Alzheimer’s and dementia and Rehabilitation
• The ability to manage administrative responsibilities with the delivery of patient care
• To work unusual hours.
