Established Organisation is looking to employ a Health Counsellor

Responsible for managing Chronic members conditions, potentially chronic members and devise treatment plans to assist members to a healthier lifestyle
Identify all compliance risks
Keep record of members interaction and follow ups done by updating system records
Develop effective relationships with stakeholders (including 3rd parties and service providers)

Requirements:
Matric / Grade 12
Clinical knowledge will be highly advantageous
Must have at least 5 years’ experience working in a managed healthcare environment working with chronic conditions and clinical issues
