A Managed Healthcare Company based in Randburg, has a Case Manager position available.
Purpose of the position:

To take responsibility for the identified patient in the Patient Driven Care system and applies clinical protocols in consultation with the members’ provider/s to optimise the patients’ level of care.
To holistically manage the identified patients future resource entitlement and associated life style interventions, to ensure adherence to care interventions.

Job Requirements:
Apply functional expertise in all areas of Patient Driven Care to ensure maximum cost effectiveness for the group and appropriate treatment for the member
Apply functional knowledge to negotiate with the treating provider to ensure that patient specific targets for optimal disease control are established and met.
Follow algorithms and protocols to drive event rates and cost down.
Take responsibility for the patient in and out of hospital i.e. Chronic disease management, acute event monitoring and management, discharge planning and life skills assistance once embarked on a programme set out by the treating health care professional (doctor, nurse , dietician , physiotherapist, biokineticists etc.)
Manage self-development and performance.

Minimum requirements:
Qualification:
Registered Nurse
Experience:
ICU Qualification or at least 2 years’ experience working in an ICU environment
The following exp will be a bonus :
Theatre Technique Qualification or at least 2 years’ Experience
Psychiatry Nursing Qualification with at least 2 years’ exp in a psychiatry environment
